Grant Renewal Application/Report Procedures:

Submit by U.S. mail:
Please send us your renewal application/report that provides the following information (font size must be 11 or larger).

  • Cover letter, on organization letterhead with contact information, that briefly summarizes the request for renewed funding (including the amount) and lists the enclosed attachments (not to exceed one page).

  • Narrative, with a header on each page identifying your organization, that addresses the following information (not to exceed three pages):

    Basic Information:
    1. Name of Organization
    2. Program Name (if applicable)
    3. Date and amount of last grant

    Organization Overview:
    1. Briefly describe the agency, include when founded, mission statement, clients served, and programs offered.
    2. Please address any significant changes that may have occurred with the organization’s overall mission, health, management, or programs since you last applied for the Stulsaft Foundation. Are there organizational issues that need to be addressed in order to continue and/or improve? What are those and how do you plan to address them?

    Progress Report:
    1. Briefly describe the program (if applicable).
    2. Evaluation: What were the specific measurable objectives that you set for the grant period (as stated in your last proposal)? Did you exceed, reach, or fall short of your objectives? What methodology did you use to acquire the data? (Please also include an evaluation report if conducted by an independent evaluator).
    3. What did you learn from the experience and what effect did it have on your organization as a whole? What were the successes, challenges, and changes? Have there been any unexpected outcomes?
    4. How were Stulsaft funds used? Do you think that this grant was instrumental in any way in your getting other funding? How so?

    Renewal Request:
    1. What are your new objectives? Are they different from last year? How and why? If there are changes in the amount of the grant request for this round, please describe them here and specify the requested amount.

  • Attachments, please also submit the following documents (these documents are not part of the three page narrative limit.)
    1. Organization budget detailing all proposed expenditures and projected sources of funding.
    2. Program/project budget (if applicable) detailing all proposed expenditures and projected sources of funding.
    3. Funder Worksheets for the organization as a whole and the program/project (if applicable).
    4. Financial statement showing actual revenue and expenses for the agency’s most recently completed fiscal year.
    5. A list of the members of the board of directors, if it does not already appear in your other materials (such as on your letterhead).
    6. Copy of the agency’s IRS 501(c)(3) tax-exempt determination letter, if it has changed since you last applied to the Morris Stulsaft Foundation.

For environmental and economic reasons, please do not use binders, folders, or other binding for proposals, audits or supporting attachments, and please do not send audio-visual materials unless requested. Thank you.
