Grant Reporting Instructions

The grant report should include the following:

  • Cover letter (not more than one page):
    • On organization letterhead with address and phone number
    • Purpose of the previous grant
    • A single contact, with phone and email information
    • A list of attachments
  • Organization and/or program goals, objectives, and outcomes (not more than one page). This document is your opportunity to establish the effectiveness of your organization and/or program. Please identify the metrics you use to measure effectiveness and the outcomes you have collected.
  • Organization and/or program change (not more than one page). Please describe any significant changes that have occurred with respect to you organization’s or program’s mission, finance, management, facilities, staffing, programs, etc. since the foundation’s previous grant.

Please save these documents in pdf format, attach to an email, and send it to Jessi Misslin, Program Coordinator, at submissions@kimballfoundation.org. You will receive an email acknowledging the receipt of the grant report in 2-3 business days. If you do not receive an acknowledgement, please contact Ms. Einhorn at weinhorn@pfs-llc.net.
